wireless / wifi cards confirmed to work in puppy

MODERATORS: This thread was specifically set aside to list wireless adapters that are compatible with Puppy. It was allowed to be hijacked by people needing assistance with their wireless devices. WHY?
Among other places in the Puppy forums, those seeking wireless assistance are now posting their questions to three forums and this thread:
1. Advanced Topics » Hardware
2. Advanced Topics » Hardware » Networking
3. Advanced Topics » Hardware » Networking » Wireless
4. Advanced Topics » Hardware » Networking » Wireless / wifi cards confirmed to work in puppy
This makes it veeery difficult to know where you should post your solicitations for assistance. It also makes this list of working adapters unnecessarily difficult to find and use.
There's a lot of useful info in the Puppy forums, so why do you allow this kind of anarchy? Hey, why not just have a free-for-all forum called 'Networking' and allow everyone to fend for themselves? It really wouldn't be much different, would it?
Why should anyone organize info like that below when it's sure to be buried within a few short weeks? Organization is our friend, people!

I was frustrated with the difficulty of finding a network adapter for my old laptop that worked with Puppy, so I consolidated the posts in this thread. Perhaps this compiled list will be useful to others.
In the future, it would be most helpful if posters provided as much information as possible, including any special steps taken to configure the device. The following could be used as a template:
Device Name & version (from package or device):
Puppy versions tested:
Interface (USB, PCMCIA, PCI, mini-PCI?):
Chipset:
Device Driver (name & source):
WiFi Protocols that work (A, B, G, N):
Security Protocols confirmed working (WEP, WPA, WPA2):
Configured with (Wizard, Ndiswrapper, Manual, Other?):
Posted by & date:
Location of Original Post:
Notes & Issues:
Look for the red highlighting that indicates WiFi devices confirmed to work with one of the official 4.xx Puppy versions.
================================
> Devices are organized in each category by their original posting date. That tends to move newer devices, and devices tested with newer versions of Puppy towards the end of the list - a help to those looking for a device that works.
